New Approach Proven to Improve HbA1c Levels for Program Participants with Diabetes
Tucson, AZ, Dec. 9, 2024— Pyx Health, the leading social health platform addressing loneliness, today announced the launch of a new program to improve condition management – providing added value to health plans and their members. The customizable program supports a wide range of conditions, including diabetes, hypertension, pregnancy, and much more.
Expanding upon Pyx Health’s existing offerings, the new program will provide support for specific conditions, including additional screenings that serve as a valuable quality measure for health plans. Members with long-term or life-changing health conditions will benefit from the support of compassionate, trained mentors and health coaches who offer companionship, techniques to reduce anxiety and stress, and help in navigating the treatment process. Pyx Health also identifies Social Determinants of Health (SDOH) needs that may impact health outcomes – such as inadequate housing, food, or healthcare access – and connects members with appropriate resources.

Pyx Health’s approach is supported by a new peer-reviewed, published study titled, Implementation of a Technology-Enabled Diabetes Self-Management Peer Coaching Intervention for Patients With Poorly Controlled Diabetes: Quasi-Experimental Case Study. The study enrolled 300 Medicaid patients with uncontrolled diabetes in a 6-month intervention powered by Pyx Health. Findings revealed that average participants in Pyx Health’s program saw over a 1-point improvement in HbA1c levels compared to those not enrolled in the program.
